Integration is the key when it comes to making sure patients get the best health care. To ensure that patients have optimal health results when they leave the hospital, Atlantic Health System offers the Transitions of Care program to patients. We focus on helping patients understand what the next steps are in terms of follow-up with their doctor, getting their medications, taking care of themselves at home, just coordinating and navigating the system. Seniors' Health Programs and Screenings


Education and prevention can keep you and your loved ones healthy. We invite you to take advantage of the programs, support groups and screenings available. Adults 65 and older who are looking to stay well with age may benefit from events labeled “New Vitality.”
 

Smoking Cessation Program
This free, six-week course will provide behavior modification strategies and coping skills to help tobacco users kick the habit.

Bereavement Support Group
Participants can receive mutual education and encouragement in this group funded by the Chilton Medical Center Auxiliary.

Ostomy Support Group of North Jersey
Patients who have had an ostomy can receive mutual education and encouragement.

Mall Walker Program
Experience a fun way to exercise indoors at the Willowbrook Mall in Wayne, NJ. Registration and a membership badge are required to participate.
Monday through Saturday, 7:00 to 10:00am
